Do you still need this home visit done? I live in Florida but grew up not far from Warsaw. My sister still lives there and works at the hospital in Warsaw as the Birthplace Manager. I could ask if she would be willing to do it if you send her whatever paperwork she would need to take with her. She isn't a member of this forum but she is a dog owner and may be willing to help. Please let me know.
