Mine are kind of boring with the dog name on the front and then our address and two phone numbers on the back.
My sister has one on her little escape artist chihuahua that says "Oh $hit, I'm lost! Please call my mom at xxx-xxxx right away because she is definitely ugly crying."

My name (first and last), dog's name, city & state (because I often travel with them to different states and Canada), phone and sometimes email. If there is room for another phone number, I add my parents' number.

Dog name on front, our name/address/phone # on back. Also embroidered collar with name/phone large enough to be seen even if dog doesn't get close enough to grab.
I don't think it would be an issue with either of my present two, but I've wondered in the past if I should put "cookie" instead of "name" on the tag. (Cookie would definitely work with these two, too!)

I have Cassie's name and my cell # embroidered into her collar. Since we are off camping and hiking all over the place, someone could still reach me if she were to ever get separated from me. Scary that we have to think of someone "keeping" a found dog.
On a lighter note, if I should become injured on a hike I could send Cassie off like Lassie to find help. They would have the number to call me

Keeping a found dog IS a problem. My crackpot neighbour tried to do just that but fortunately was finally persuaded to give it back, actually to OSPCA as it needed medical attention. The point is, she didn't know that dog's name; the dog happily responded to the new name she gave it. In such a case, with some convoluted backwards thinking, I think I'd at least want such a person to call my dog by the name I gave him. Then, perhaps, wild chance, someone would say, Oh they have a new dog two doors down, it looks like your lost poster and they call it the same name you do. Or else such people would be smart enough to change the name anyway.
Any dog (and cat) I've found also responded to what I called it, Pretty Girl or Pretty Boy or Hey You.

It's very scary to think that someone would keep a lost dog, even if it had tags etc. and was micro chipped. I guess these things only work if a trustworthy person found your dog.
We all need to be very careful with our dogs, so they don't get lost in the first place.
