    Ectopic Ureter(s)

    I have received Dozer's CT Scan results today
    The CT Scan revealed - "abnormal termination of the right ureter at the junction of the trigone and proximal urethra (ureteral ectopia) and indiscernible site of insertion of the left ureter. Mild bilateral hydroureter and mild pyelectasia". His ureters measured 3mm in dilation each, there was mild distention of the renal pelves - right renal pelvis was 3.8mm in diameter and the left renal pelvis measured 5.1mm in diameter. I am not 100% understanding of what all of this means other than he has a right ectopic ureter for sure, the left ureter attachment was hidden by stool therefore the attachment not visualized, but due to the dilation it is assumed it also is Ectopic.
    He has been improving lately, not peeing as frequently (down from 18 times a day to 6-7 times a day) or dribbling as much (or not even at all some days) and I had almost been tempted to cancel the CT Scan as I was assuming all of his symptoms were related to the UTI he is currently being treated for. The Vet put him on Propalin about two weeks ago which has almost completely stopped his urinary incontinence. He back to sleeping all night and is able to get up from his sleep with little to know dribbling. The Vet instructed me to stop the Propalin as he felt the Propalin would contribute to urine being held in the Ureters and potentially reaching the kidneys.
    Does anyone on the lab board have experience with Ectopic Ureters?? Is surgery the only route to go?? Can it be treated with medication....or do all dogs with Ectopic Ureters require surgery to prevent damage to their kidneys from urine back flowing up the ureters?? I obviously want to do what is best for Dozer. He has a consult scheduled on April 5th for a Cystoscopy and Ultrasound to determine if he is a candidate for minimally invasive laser surgery for Ectopic Ureters. If he is he would have the procedure done the next day. There is a chance his ureters are intramural and that would entail a more invasive approach. The Veterinary Hospital where he has been referred is almost 6 hours hours from our home, so everything will happen quickly and be done at the same time.

    I appreciate any experience, advice, knowledge that the lab board has to offer....I just feel so overwhelmed

    Aww, so sorry to hear it's not something simple. I have no advice to give but I'm sure someone on here will be able to provide something. At least you know now exactly what it is. Dozer is lucky to have such a great mom! Good thoughts your way!

    One of our old members, Daisy and Miia, has a dog Paige who underwent surgery for an ectopic ureter. The surgery was successful and last we heard, Paige was doing well. Sadly Nancy, her owner, comes here very infrequently now. I suspect surgery is the only solution for the condition but the vet obviously has to confirm that. I’m sorry to hear this may be the cause of his problems.

    That pup was one of Julie's puppies....I couldn't remember her name! Got her introduced to Julie! Sadly this is just one of those stupid congenital defects that happens. Hopefully Michelle can get some good input for the next step.

    I have seen several over the years and one of my pups had one. The problem with ectopic ureters is that it is an anatomical defect. Most of the time the ureter enters below the trigone (sphincter) of the bladder instead of in the bladder itself. This allows the urine to bypass the bladder and continually dribble out the ureter. There is no effective treatment as most urinary leakage therapy focuses on tightening the bladder sphincter. The condition calls for surgical repair. The ultrasound for your dog suggests that both ureters are likely involved even though they could not see the insertion of the one ureter. I say this because it states bilateral hydroureter and bilateral renal pelvis dilation. This suggests that there is improper urine flow through both ureters causing a back up of urine, which causes the ureters to enlarge in diameter as well as the renal pelvis in both kidneys. Sort of like a beaver dam backing up a creek, forming a pond. Both ureters are open and flowing with only partial obstruction, likely due to the location at the sphincter/proximal urethra where there is no ability for expansion like the bladder itself. Most likely, both sides will need to be surgically repaired (re-attached correctly to the bladder) to avoid permanent damage to the kidneys due to urine back up (renal pelvis dilation). Sorry to inform you of this but it sounds like no one sat down and explained it to you. My pup had the surgery and did fine. Good luck.
