    Thank you everyone, for best wishes and suggestions. Biopsy was done on Wednesday, results should be back in 3-7 days. Folliculitis was another differential DX in the pemphigus info I was looking at. Hope it's that and not autoimmune....

    Path report finally came back, vet called while I was in S. Carolina. It showed as ringworm, fungal infection! Which is why the ABX did not help, and the pred only knocked down the inflammation on the open wound, and did nothing for treating the fungus. The oral meds for this are nasty, so we are going to hold off and do topicals only at this point. Annie needs chlorhexidine baths every 3 days for a couple of weeks, and I am getting some sort of anti-fungal wipe for the spot on her snout. SO glad I pushed for the biopsy, ringworm was never mentioned in any discussions w/ the vets. Anyway, now I have an answer and we can treat correctly. Vet also mentioned it is zoonotic, and can be passed to Rudy (and humans), although I have seen nothing on him yet. Has anyone else here dealt w/ ringworm skin issues? Thanks everyone, it has definitely been a learning experience

    Glad it was fungal. I've had dogs on oral anti fun gals, ketoconazole and the like and while the labeling is scary, we had no issues. It might be the best was to eradicate it and a short term course that is monitored should be fine. We had our allergy queen on pulse therapy (Derm vet prescribed) for chronic environmental allergies and she lived to be 15.

    You need a biopsy to confirm pemphigus in a dog. Too many other dermal lesions can be similar. If it is pemphigus, I would question if there was a correlation with the lesion developing after giving a vaccination. There has been research that suggests that it often occurs as a symptom of vaccinosis. The picture looks as though there is an infectious component as well. Might get a culture as well.
