The vet might assume the OP is doing everything right or the OP might be misquoting the vet. Six months is about the time, in my experience, that puppies really get it and are able to physically hold it long enough to let you know they gotta go and then wait a few minutes. Before then, I find that puppies will let you know too late, then run to the door while peeing or run to the door and pee before you can open it. So, you gotta pay very close attention and make some assumptions for them.
